Annual Enrollment for health, flexible spending accounts and supplemental benefits will be held October 1, 2014 - October 31, 2014. During this period, employees will be able to enroll, cancel or make changes to their health and voluntary insurance plans. All changes made during the annual enrollment period (October 1, 2014 - October 31, 2014) will be effective January 1, 2015.

2015 Health Plan selection resources:
Employees Enrolled in an OGB Heath Plan: BCBS HMO, PPO, CDHP, or Vantage Home HMO
Action is REQUIRED for all current OGB Health Plan participants
This year, OGB health plan participants are REQUIRED to make a selection during the annual enrollment period. If you are currently enrolled in an OGB health plan and do not make a selection by the end of the enrollment period you will be moved into the new OGB Pelican HRA 1000 plan and will not have a chance to change plans until the next open enrollment period.
Employees Enrolled in LSU First
If you wish to remain in the same LSU First option (option 1 or option 2), no action is required on your part during annual enrollment. Action is only required if you want to change your health plan for the 2015 plan year (January 1, 2015 - December 31, 2015).
2015 Supplemental Benefits will be administered by UnitedHealthCare in 2015.
All plan participants will be automatically re-enrolled in the same plan with UnitedHealthCare. No action is needed if there is no change.

Long Term Care will continue to be administered by Unum in 2015.
Flexible Spending Accounts will continue to be administered by Boon Chapman in 2015.
Boon-Chapman will continue to be the claims administrator for the LSU Flexible Spending Accounts for the 2015 plan year. This program will allow employees to put tax-sheltered dollars in an account for out-of pocket medical and/or dependent care expenses that are incurred from January 1, 2015 through December 31, 2015 for the employee and any eligible dependents.
Employees are able to contribute up to $2,500 into a Healthcare Spending Account and up to $5,000 (per household) into a Dependent Care Spending Account. The monthly administrative fee will remain at $5.10 for 12 month employees and $6.80 for 9 month employees, and includes the debit card option for both the Healthcare and Dependent Care spending accounts.
Please note: If you are currently participating in a Health Care and/or Dependent Care Flexible Spending Account(s) you must re-enroll via your myLSU account during this annual enrollment period if you wish to participate for the 2015 plan year. Failure to re-enroll in these plans will result in no FSA benefit for the 2015 plan year. Current plan participants should continue to use the same debit card for the new plan year. Employees cannot participate in the HSA 775 and a Flexible Spending Account.
**A confirmation page will appear if your enrollment was successfully submitted. If you do not receive a confirmation page, your enrollment was not processed and you will need to re-submit. Please save a copy of the confirmation page for your records, as we will not allow any enrollments past October 31, 2014.
