Annual Enrollment 2013
(October 1, 2013 - October 31, 2013)
It’s Annual Enrollment time again. All elections made during the annual enrollment period (October 1, 2013 – October 31, 2013) will be effective during the plan year of January 1, 2014 – December 31, 2014.
During this Annual Enrollment period, employees can enroll in or make changes to health, dental, vision, premiums only plan and flexible spending accounts, but HRM encourages all employees to review all benefits and make selections/changes consistent with your individual needs. LSU offers a variety of health and supplemental benefits including: dental, vision, tax saver flexible benefits, term life, accidental death and dismemberment, long term care and long term disability insurance.
**If you wish to remain in the same health plan, no action is needed.
As usual, you will be able to change health plans electronically via myLSU.
LSU will also require electronic enrollment via myLSU for Healthcare and Dependent Care Flexible Spending Accounts.
*Both portals will be available from October 1 – October 31. All health plan changes and flexible spending account enrollments must be completed during this period for a January 1, 2014 effective date.
During this period employees can enroll, cancel and make changes to the following benefits:
HEALTH PLANS OFFERED (JANUARY 1, 2014 – DECEMBER 31, 2014)
- PPO administered by Blue Cross Blue Shield
- Consumer Driven Health Plan administered by Blue Cross Blue Shield
- HMO Plan administered by Blue Cross Blue Shield
- LSU First Health Plan
- Medical Home HMO administered by Vantage (available only for employees who live in designated regions)
HMO and PPO Health Plan (BCBS)
The HMO and PPO plans will continue to be administered by Blue Cross Blue Shield and will continue to have the same benefit structure. Effective January 1, 2014, the HMO and PPO prescription drug benefit will be administered by MedImpact.
*HMO and PPO members will receive new ID cards in the mail prior to January 1, 2014.
*If you are a current member of the HMO or PPO plan and you wish to remain in the same plan effective January 1, 2014, no action is required.
Consumer Driven Health Plan (CDHP-HSA) (BCBS)
The Consumer Driven Health Plan will continue to be administered by Blue Cross Blue Shield and will continue to have the same benefit structure. The Health Savings (HSA) will remain an option for participants under the CDHP plan. The HSA option will allow employees to make pre-tax contributions to a savings account. The HSA can be used to offset the deductible and the out of pocket medical expenses. The state will contribute $200 to each member’s HSA and will match up to $575 towards plan member’s contributions.
*Please note employees who participate in the HSA option, will not be able to participate in the Flexible Spending Account (FSA) program for the 2014 plan year. Employees must have exhausted their (FSA) Healthcare Spending Account as of December 31, 2013 to be eligible for the HSA account and enroll for January 1, 2014.
Medical Home HMO (Vantage)
There have been some minor plan design changes to the Vantage HMO plan. Please see the health plan comparison sheet.
LSU First Health Plan
The following LSU First Plan Benefits will remain the same for 2014:
- You will not incur any Out-of-Pocket expenses for Covered Services from First Choice Providers and/or Generic Prescription Drugs.
- Your Health Reimbursement Account (HRA) will remain the same (this is the amount of your Deductible that LSU pays on your behalf).
- Your Remaining Deductible (the part you pay) will remain the same.
- Your Co-Payments for Brand and Specialty prescription drugs will remain the same.
*LSU First members will receive new ID cards in the mail prior to January 1, 2014
*If you are a current member of the LSU First Health Plan and you wish to remain in the same plan effective January 1, 2014, no action is required.
The following changes will be implemented for the 2014 plan year (January 1, 2014 – December 31, 2014):
The LSU First Health Plan will utilize a Nationwide Network through Aetna Signature Administrators.

Co-Insurance/Out of Pocket Maximum
The Out-of-Pocket Maximum applicable to Medical Expenses will increase by $500 for Employee Only, $750 for Employee+Spouse and Employee+Child, and $1,000 for Family coverage. This does not come into effect until you have exhausted your HRA and have met your deductible.
New Out-of-Pocket Maximums (does not include Prescription Drug Co-Pays or Deductible amount) for In-Network Providers are as follows:
Complimentary $25,000 Life Insurance Policy
The complimentary $25,000 Term Life Insurance and Accidental Death and Dismemberment offered through the LSU First Health Plan will be discontinued effective January 1, 2014.
The Dental Insurance will continue to be administered by Dearborn National. There will be no plan design changes; however, there will be a slight premium increase for the 2014 plan year.
The Vision Insurance will continue to be administered by Davis Vision. There will not be any premium increases or plan design changes for the 2014 plan year.
HEALTH AND DEPENDENT CARE FLEXIBLE SPENDING ACOUNTS
Boon-Chapman will continue to be the claims administrator for the LSU Flexible Spending Accounts for the 2014 (January 1, 2014-December 31, 2014) plan year. This program will allow employees to put tax-sheltered dollars in an account for out-of pocket medical and/or dependent care expenses that are incurred from January 1, 2014 through December 31, 2014 for the employee and any eligible dependents.
Employees are able to contribute up to $2,500 into a Healthcare Spending Account and up to $5,000 into a Dependent Care Spending Account.
*Please remember the IRS “Use it or Lose It” provision when electing your annual target.
The monthly administrative fee will remain at $5.10 for 12 month employees and $6.80 for 9 month employees, and includes the debit card option for both the Healthcare and Dependent Care spending accounts.
**A confirmation page will appear if your enrollment was successfully accepted. If you do not receive a confirmation page, your enrollment was not processed and you will need to re-submit. Please save a copy of the confirmation page for your records, as we will not allow any enrollments past October 31, 2013.
**Please note if you are currently participating in a Health Care and/or Dependent Care Flexible Spending Account(s) you must re-enroll via your myLSU account during this annual enrollment period if you wish to participate for the January 1, 2014 – December 31, 2014 plan year.
Failure to re-enroll in these plans will result in no FSA benefit for the January 1, 2014 – December 31, 2014 plan year. Current plan participants should continue to use the same debit card for the new plan year.
LONG TERM CARE and LONG TERM DISABILITY
The Long Term Care Insurance Plan administered by UNUM and the Long Term Disability Insurance administered by the Hartford are only guaranteed to employees that enroll within their first 30 days of hire. If you did not enroll within the first 30 days of hire, you can apply via medical underwriting at any time throughout the year.

HARTFORD SUPPLEMENTAL LIFE INSURANCE
The Hartford will continue to administer the LSU System Voluntary Life Insurance Plan. During annual enrollment, current participants in the plan will be allowed to increase their employee coverage in $10,000 increments by one time their annual salary, on a guaranteed issue basis, not to exceed $500,000.
*If you elect to increase your coverage during annual enrollment, please note that the new application will replace any other application(s) currently on file, so be sure to write the entire amount of life insurance desired for you and your dependents effective January 1, 2014, as well as beneficiary designations.
*Please note that the life insurance plan does not allow you to increase your spouse’s coverage at this time without medical underwriting.
If you are not a current participant in this plan, you can apply for this benefit at any time throughout the year subject to medical underwriting.

Please note that it can take several weeks for the Hartford to notify you whether or not you have been approved. No payroll deductions will come out of your paycheck until LSU receives and approval letter.
ANNUAL ENROLLMENT MEETINGS
The Office of Human Resource Management will be hosting annual enrollment meetings during the month of October to discuss updates and provide information on all of the health and supplemental benefit plans.
The Office of Group Benefits (OGB) will not be hosting any on site meetings during annual enrollment for employees this year. They will however be providing LSU with an online video discussing changes for the upcoming year.
