The PPO Plan is a form of managed care that has contracts with doctors, hospitals and other providers who offer medical services on fee-for-service basis (click here to search for providers, doctors and hospitals).
• Members may use any provider within or outside of the PPO network, but have a financial incentive to stay within the network.

At times you may have to pay out of pocket for medical services or for prescriptions, particularly if you have received services from a non-network provider. In the event that you do pay out of pocket for services, you can be reimbursed for these services. You will need to complete and submit a claim form and a copy of the invoice to the appropriate carrier. You may submit a claim form (see below) along with a copy of the invoice to your health insurance carrier. BCBS will then reimburse you according to the plan's guidelines.

Effective January 1, 2013, the Office of Group Benefits will no longer be the plan administrator for the PPO plan. The contract to administer the PPO plan was awarded to Blue Cross Blue Shield.The PPO plan will continue to have the same benefit structure, but will now include a nationwide network through Blue Cross Blue Shield.
