The LSU First Health Plan is a fully-funded self-insured plan. Utilizing CIGNA as the Claims Administrator and the Medical Manager, Express Scripts as the Pharmacy Benefits Manager, and Verity HealthNet for local and First Choice Provider network administration.

 

*Nationwide Coverage

 

• Plan members control which doctors they see.
• In most cases, no co-payments or deductibles are due on site.
• Plan members receive a Health Reimbursement Account (HRA) - these funds are used to cover allowable services. HRA balances roll over from one plan year to the next.
     - The First Choice Provider Network provides 100% coverage once HRA funds have been exhausted.
     - 100% coverage for generic medications once HRA funds are exhausted.
• Plan members have their pick of three networks within LSU First CIGNA's Open Access Plus Only Network (national network), the Verity Health Net (statewide network and surrounding areas), and the First Choice  Provider Network (statewide).
• A current listing of network providers for the LSU First plan is accessible at any time on the LSU First website. If you do not have access to a computer, visit any public library for free internet access.

There are two options of Plans through the LSU First Health Plan: Option 1 and Option 2. The difference between Option 1 and Option 2 is the Member Responsibility is higher under option 2. The monthly premium is lower as a result of the higher out-of-pocket responsibility of the Member under Option 2. Option 2 may be a good choice if you have rollover HRA dollars from a previous Plan Year or if you know you will have minimal healthcare costs during the Plan Year.

Prescription Drug Coverage for Brand Name and Specialty Drugs
The Plan has been revised for Members who utilize Brand Name and Specialty drugs once their deductible has been met. As opposed to these drugs being covered at 90% of the contract cost by the Plan and the Member being responsible for 10% of the contract cost, Members will be responsible for paying a $40 co-pay for a 30-day supply of Brand Name drugs and a $120 co-pay a 30 day supply of Specialty drugs. Furthermore, these co-pay amounts will not count towards the out of pocket maximum.
*Please note that generic drugs will continue to be covered at 100% throughout the plan.

 

Selective Home Delivery ****UPDATE****
Beginning January 1, 2013, Members who utilize maintenance drugs will have the option of whether or not they want to participate in then Express Scripts Home Delivery program. All employees will be required to contact Express Scripts at 1-866-929-5781 to select whether or not they wish to participate. Employees will be allowed two refills before they are required to select. In the event that the employee does not select, you will be required to pay the full price of the prescription until a decision has been made and Express Scripts has been notified. Please click

For newly eligible employees with an effective date after January 1st, the Deductible and HRA Contributions will be pro-rated, based on the remaining number of months in the Plan Year. Click here for the New Hire Proration Guide.

 

 

At times you may have to pay out of pocket for medical services or for prescriptions, particularly if you have received services from a non-network provider. In the event that you do pay out of pocket for services, you can be reimbursed for these services. You will need to complete and submit a claim form and a copy of the invoice to the appropriate carrier. You may submit a claim form (see below) along with a copy of the invoice to your health insurance carrier. LSU First/CIGNA will then reimburse you according to the plan's guidelines.

Complimentary Term Life insurance and Accidental Death & Dismemberment Policy

LSU First provides all participating employees and retirees with a term life insurance policy in the amount of $25,000 as well as an accidental death and dismemberment policy in the amount of $25,000 at no additional cost to the employee. Employees are required to complete and submit a beneficiary designation form to the LSU Benefits Service Center.
