In order to ensure a smooth transition into your role as an LSU employee, there are certain pieces of information that must be collected. Details regarding your direct deposit choice, acknowledgement of university policies and the completion of the mandatory Immigration Reform and Control Act and I-9 form are just a few examples of the information that can be gathered by clicking on the link provided in the email which accesses an Employee Data Sheet.
You will be required to log on to myLSU with your PAWS ID and password to access the Employee Data Sheet. If you have not yet received your PAWS ID and password, please contact the HR Contact of your department.
Directory Information: Please provide all the requested information in this section.
Relative Information: Please indicate if you have any relatives who are employed with LSU, or to indicate you have no relatives working at LSU. See Policy Statement 25, entitled "Nepotism," for further information.
Race/Ethnicity Information: Please complete the race/ethnicity information in compliance with the Equal Employment Opportunity requirements.
Veterans: Please answer Yes or No. If yes, please continue and complete this section.
Prior Service: This section includes Selective Service and prior service of any Louisiana retirement system. This section is required to be completed for all employees.
Direct Deposit: Employees are required to Direct Deposit payroll checks. Please complete your banking information in this section.
I-9/Employment Eligibility Verification: Please enter and SAVE all information in order to continue to the next section of the Information Sheet. You are required to bring acceptable documents to the HR Contact of your department. Acceptable documents can be found on page 4 of the I-9 form.
Tax Withholding: In order to complete your appointment, payroll tax information is required. Please complete the Federal W-4 Withholding and the State L-4/L-4E Withholding documents. If this information is not provided, the payroll office is required by law to withhold at the highest Single tax withholding rate.
Loyalty Oath: Click “Agree” if you acknowledge that you have read and agreed to the oath as stated.
Drug Policy: LSU has a strict policy on drug use and employment at the University requires acknowledgement of that policy. Please electronically sign the acknowledgement where indicated (by submitting the form, you electronically sign).
Leave Election: Eligible employees are allowed to choose between two methods of accruing annual leave. Read about the different options available to you and to make the election. Your election is IRREVOCABLE.
Insurance/Flexible Benefits: Employees eligible for benefits have 30 days from the date of hire (or date of eligibility) to complete enrollment documents and submit them to the Office of Human Resource Management. Please read the acknowledgement and indicate your understanding of the benefits enrollment deadlines.
